What, if any, previous climbing experience does the child or children have?

Please read the following statements and check the boxes to agree.

I, the parent/guardian, understand that my child's participation in rock climbing and indoor climbing activities with Mountain Craft, whilst supervised, still carries inherent risks of personal injury or death. I accept these risks on behalf of my child(ren) and acknowledge that Mountain Craft and its instructors/agents cannot be held responsible for any loss, damage, injury, or death arising from their participation in these activities, unless caused by negligence on the part of Mountain Craft.

I consent to Mountain Craft, their instructors or agents to provide emergency medical & first aid care to the named child(ren) if I am unable to give consent at the time.

I understand that Mountain Craft holds public liability insurance. However, I am advised to consider taking out my own personal accident insurance to cover my child(ren) for personal injury, property loss/damage, or activity cancellation, as Mountain Craft's insurance does not cover these eventualities for participants.

I confirm that I am the parent or legal guardian of the child(ren) listed above, and I will be present for the entire duration of the activity.
